New-patient paperwork is available to complete online before your first appointment. A few minutes on the couch beats a clipboard in the waiting room, and it means we can get started the moment you walk in.
When you book, our front desk makes sure you have everything you need to finish ahead of time. If anything is unclear, call (201) 345-3637 and we'll walk you through it.
What to bring
- Your insurance card — we'll handle the verification
- A list of any medications your child takes
- Any questions you have, big or small
- A comfort item, if it helps your child feel at ease
